    • The aim of this study was to explore aspects of the meaning of home as experienced by very old single-living people in Sweden. A grounded theory approach was used, and interviews were conducted with 40 men and women aged 80-89. The findings indicate that home has a central place in the lives of very old people because it is where they live and spend so much time. The significance of the home is based on the fact that it means so many different things to the participants. The theme comprises two key categories: home means security and home means freedom. Each of these has three sub-categories. In home means security, these are: living in a familiar neighborhood, everything functions, and having memories to live on. Home means freedom comprises a place for reflection, a social meeting-point, and leaving your own mark. Home is part of the environment and influences the meaning and selection of activities that very old people decide to engage in. When occupational therapists prescribe assistive devices or recommend changes in the home environment, they must be very well aware of and reflect on what home means to their clients and base their measures on that.

    • The aim of this study was to explore independence in the home as experienced by very old single-living people in Sweden. A grounded theory approach was used and interviews were conducted with 40 men and women aged 80-89. Data analysis revealed the core category "Home as a signification of independence" with two main categories: "Struggle for independence" and "Governing daily life". The findings showed that home is strongly linked to independence, and being independent is extremely valued. Explicit descriptions of the ageing process as an individual process of changing living conditions within the home emerged from the findings. Hence, the ageing process influences the participants' perception of themselves as independent persons. Along the ageing process the participants' view of independence changed from being independent in activity performance without help from others to experiencing independence in being able to make autonomous decisions concerning daily life at home. Consequently, there is a need to develop strategies to support very old people in staying as active and independent as possible in their own homes. In addition, since the findings highlight that independence is a complex construct, there is a need for conceptual differentiation between independence and a construct often used synonymously, namely autonomy.

    • The aim of this study was to increase our understanding of the multifaceted phenomenon of home and health, by exploring processes underlying these dynamics when living at home in very old age. A qualitative meta-synthesis was used on four complementary qualitative studies, based on data from the Swedish ENABLE-AGE In-depth Study, focusing on very old, single-living people's experiences of home in relation to participation, independence and health (N = 40). Over time, very old people manage increased frailty by means of three intertwined processes concerning changes in everyday life: The turning points, The struggle and The negotiations. The findings show that these processes take place through the context of the Homelike functional home and a state of mind labelled, The awareness of frailty. The results can be useful in constructing and implementing guidelines for more holistic approaches to housing provision for senior citizens. Further, the study delivers a deeper understanding about how very old people live their lives, useful for practitioners as well as it contributes to theory development within the context of ageing and environments.

    • The aim of this article is to introduce an integrative and more comprehensive approach to understanding and measuring perceived housing in old age. First, four conceptual domains of subjective housing were introduced, based on the assumption that each of the domains brings a unique perspective to the understanding of perceived housing: housing satisfaction, usability in the home, meaning of home and housing-related control beliefs. Second, relationships between the proposed domains were empirically examined using correlative analysis, factor analysis and structural equation modelling (SEM) techniques. Cross-cultural similarities and differences in the observed empirical relations were then analysed across three Western European countries. Data were drawn from a sub-sample of the participants in the European ENABLE-AGE Project amounting to 1223 old adults aged 80-89 years and living alone in their private homes in Swedish, British, and German urban regions. The ENABLE-AGE data set has the advantage of containing measures related to all four domains of perceived housing which are the focus of this paper. The results of exploratory and confirmatory factor analysis as well as of the SEM give empirical support for the usefulness of the theoretically proposed four component model of perceived housing. Furthermore, multi-group analysis supports the assumption of similarity of perceived housing among older people living in the different countries. (c) 2006 Elsevier Ltd. All rights reserved.

    • This study aimed to capture and operationalize the concept of participation among people who are very old (age > 80) and to investigate whether and how aspects of housing and neighborhood environments relate to participation in very old age. Two dimensions of participation--performance-oriented participation and togetherness-oriented participation---emerged from an earlier qualitative study and were operationalized and validated in this study with quantitative variables. Both objective and perceived housing and neighborhood aspects were significantly related to, in particular, performance-oriented participation and, to a lesser extent, to togetherness-oriented participation. Most interventions in occupational therapy are accomplished within the home for very old people, but our results imply that there are potential interventions to accomplish on a societal level as well. In this respect, our study contributes to the knowledge in the field and has the potential to support the development of novel occupational therapy interventions targeting society at large.

    • The main aim of this study was to evaluate an occupational therapy based intervention aimed at increasing the accessibility to buildings with public facilities in a Swedish town centre, which targeted physical environmental barriers. The intervention was occupational therapy advice based on the environmental assessments of each facility, given to the facility owners in order to ease the removal of environmental barriers. Another aim was to elucidate the attitudes towards and the practical obstacles to the implementation of accessibility measures among public facility owners. Systematic on-site observations of environmental barriers were administered in five buildings with different facilities, at baseline and at follow-up 18 months later, and were complemented by semi-structured interviews with the facility owners at follow-up. At baseline, environmental barriers were found in all five facilities, such as at entrances, and at follow-up only minor improvements were identified. Two of the facility owners had made use of the occupational therapy advice, but the results also revealed scarce knowledge of or negative attitudes towards accessibility measures. Much remains to be done when it comes to attitudes towards the inclusion of people with disabilities. Active occupational therapy, as described in this study, can influence the situation only to a limited extent. Nevertheless, the results demonstrate that there is knowledge to be gained through this kind of approach.

  • The Housing Enabler Screening Tool: Feasibility and Interrater Agreement in a Real Estate Company Practice Context

    • The ambition underlying this study is to provide a valid and efficient screening tool targeting accessibility problems in the ordinary housing stock. The study aims to test the feasibility and interrater agreement of a screening-tool version of the Housing Enabler in a real estate company practice context. Two rater pairs administer the screening in 35 apartments in a Swedish municipality. The overall agreement for the rater pairs was 83% and 78%. For 29 of the 61 items of the tool, the agreement was > 80% for both rater pairs; for 8 items, it was < 80%. The findings show that the tool is feasible for use in a real estate practice context but that there is a need for more rater training and an instrument manual. This study contributes to the development of research-based strategies for the identification of accessibility problems in ordinary housing, with the long-term aim to increase accessibility in the housing stock, ultimately supporting activity and participation among senior citizens.

  • Accessibility and usability in housing: construct validity and implications for research and practice

    • Purpose: The aim of this study was to validate the conceptual definitions of accessibility and usability, and to explore differences between objective accessibility assessments and subjective ratings of usability in different client groups. Method: The Housing Enabler and the Usability in My Home instruments were used for 131 persons above 18 years of age, living in ordinary housing and receiving a housing adaptation grant. Covariation between accessibility in four different housing sections and three different usability aspects were explored, for the total sample and for six sub-samples reflecting person-environment-activity transactions or demographic factors. Results: Significant correlations were found in the total sample, among clients aged 75-84, women, clients living alone, as well as among clients with high dependence in personal and instrumental ADL and in outdoor activities. Subjective usability evaluations of activity aspects and physical environmental aspects were correlated to accessibility indoors and outdoors, while personal and social aspects of usability were correlated to outdoor accessibility. Conclusions: Accessibility and usability are concluded to be different but related concepts. The results indicate that e. g. age, civil status and ADL dependence affect how clients assess aspects of their housing situation. For efficient planning and evaluation of housing adaptations, assessment of housing accessibility, usability, and dependence in ADL is recommended.

    • PURPOSE: Owing to physical inaccessibility persons with mobility restrictions and other functional limitations often face problems in public environments, leading to restrictions in activity and participation. To investigate general accessibility and perceived problems of accessibility to the public environment in a town centre, as well as visiting preferences to public facilities, among teenagers with functional limitations. METHODS: An interview questionnaire specific to a south Swedish town was constructed and used with 33 Swedish teenagers with functional limitations. RESULTS: To a varying degree, all 33 teenagers commented on accessibility problems, e.g. concerning uneven surface material outdoors, steps at entrances, heavy doors and restricted space indoors. The results also indicated that teenagers with functional limitations to a high extent want to visit the same environments as other teenagers, but that it is often impossible owing to accessibility problems. Furthermore, because of accessibility problems, many of the teenagers were dependent on personal assistance. CONCLUSION: Inaccessibility results in dependence, which might affect personal development negatively, and much effort are required in order to ensure activity and societal participation. Efficient priorities in public environment accessibility matters and discussions with the actors involved require valid and reliable data on local accessibility problems.
